SIP trunk shows outgoing status but not incoming

Is this a known issue?  I'm running the latest FOP in FreePBX 2.2.1/Trixbox
2.2.  I have a SIP trunk provisioned for a local SIP provider.  For outgoing
calls, FOP show status of green before the other end answers and Red when
answered.  This is expected behaviour I assume.  For incoming calls, it
always shows green no matter what.  Is this a known issue or possible
configuration issue or could it be something with the sip provider.  

My sip.conf is as follows:
