I think I got it.
It's not documented anywhere but I had to edit amportal.conf and change the
following entry:
"AMPWEBADDRESS=mylocalip"
Change to
"AMPWEBADDRESS="
Also, the default port setting in "manager.conf" is "5038" which I think I
also have to forward on the firewall otherwise I get flashing red on the
extensions in FOP. I could probably change the port in "manager.conf" to
port 4445 instead of forwarding another port in the firewall but this is all
speculation.

Check your linux firewall rules. Try to telnet to port 4445 and see if it
fails or connects.. Enable the debug icon in the flash panel (changing
op_style.cfg) and open the debug window in the flash side.
