Monitoring / Barge In on agents

Hi,
I installed FOP and it works perfectly except monitoring agents which is the
feature I
installed FOP in the first place. Monitoring normal calls works just fine ,
however when
I try to monitor an agent, my phone rings and is immediatelly disconnected when
I lift
the handset. Asterisk says:
> Channel SIP/BT101-085804d8 was answered.
== Starting SIP/BT101-085804d8 at sip_phones,,1 failed so falling back to
exten 's'
== Starting SIP/BT101-085804d8 at sip_phones,s,1 still failed so falling back
to context 'default'

'SIP/BT101-085804d8' sent into invalid extension 's' in context 'default', but
no invalid handler
-- Channel 0/25, span 1 got hangup request
I have tried 0.27-snap and 0.25 but the behaviour is the same.
What am I doing wrong here ?
